What is Implant Removal and Capsulectomy?

Implant removal and capsulectomy is a procedure to remove breast implants and, when necessary, the scar tissue (capsule) that forms around them. Over time, implants may no longer suit your lifestyle, aesthetic goals, or health needs. This surgery allows you to return to a more natural breast shape or prepare for alternative treatments, such as fat transfer or new implants.


Why Choose Implant Removal or Capsulectomy?

There are several reasons someone may choose to have their implants removed, including:

  • A desire to return to a natural breast appearance

  • Changes in lifestyle or personal preference

  • Capsular contracture (tightening of scar tissue)

  • Implant rupture or concerns about implant integrity

  • Discomfort or changes in breast appearance over time

Many women also choose removal as part of planned long-term maintenance, since implants are not lifetime devices.

How the Procedure Works

The operation is performed under general anaesthetic. The type of surgery depends on your specific needs:

  • Implant removal only: The implant is taken out through the original scar where possible.

  • Capsulectomy: Some or all of the surrounding scar tissue is removed if it is causing tightening or distortion.

  • En bloc capsulectomy: In selected cases, the implant and capsule are removed together as one piece.

After removal, the breast shape is carefully reshaped. Some women choose additional procedures, such as uplift or fat transfer, to improve contour and fullness.

What to Expect After Surgery

You can expect swelling, bruising and a feeling of tightness for the first few days. A supportive postoperative bra should be worn for several weeks to help the breasts settle. Light activities can usually be resumed within a week, while more strenuous exercise should be avoided for 4–6 weeks.
Your breasts will change gradually over the following months as swelling settles and the tissues heal.


Important Considerations & Long-Term Information

Once implants are removed, the appearance of the breasts will depend on your natural tissue, skin elasticity and how long the implants were in place. Some women may have looser skin or reduced volume — this is normal after implant removal.
Options such as a breast uplift or fat transfer can be discussed if you wish to refine shape or restore fullness.
Most women feel relief and satisfaction after removal, especially if implants were causing discomfort or cosmetic concerns.

1What does surgery involve?
Your implants are removed via the same incision used to insert them. In some cases the scar may need to be longer. If the capsule (scar tissue around the implant) is also being removed this will be done at the same time and drains will be inserted. The skin will be stitched closed and a dressing placed.
2How can I prepare for surgery?
You will need to buy a post-surgical front-fastening bra. You should buy a selection of sizes. You will need to wear the bra day and night for 6 weeks after surgery to reduce fluid build-up. You should stop smoking 6 weeks prior to surgery and follow any advice about your medication.
3What are the risks of surgery?
These will be discussed in the clinic. Details of your implants will be taken when they are removed and the information entered into the Breast and Cosmetic Implant Registry.
4What is the post-operative recovery?
Your wounds will usually be healed in 2 weeks. Occasionally areas can take up to 6 weeks to heal. You will need to wear your post-surgical bra day and night for 6 weeks.
5What about mammograms?
When having mammograms in the future you should let them know you have had surgery. There is a higher chance you will be recalled to check an area in the breast but there is no increased chance of getting breast cancer after this surgery.
6How will my breasts look after having the implants removed?
Your breast tissue will have been stretched from having implants inside. It is difficult to predict to what extent the skin and tissues will ‘recoil’. You should be prepared to have some loose skin and a greater degree of ptosis (droop) of the breast than you did with the implants in place.
